In Colorado, Genoa Healthcare operates 14 pharmacies and is excited to expand telepsychiatry programs to CMHCs, FQHCs, primary care groups, and substance use treatment facility partners.

GETTING STARTED

Who can be a telepsychiatrist for my clinic?

Psychiatrists & APRNs: Any psychiatrist (MD/DO) or Advanced Practice Registered Nurse
(APRN) with a state license in Colorado can provide care via telepsychiatry.

In addition, Colorado adopted the interjurisdictional Compact of the Association of State and Provincial Psychology Boards (PSYPACT), an interstate agreement that allows physicians to obtain medical licensure in multiple states through a single application process.

  • Goes into effect on January 1, 2020 unless a seventh state enacts the Compact before then.

How does reimbursement work for telepsychiatry in Colorado?

Each telepsychiatry session generates two billable codes:

  • The standard CPT codes for outpatient, psychiatric services.
  • An originating site facility fee (Q3014) that allows clinics to receive reimbursement for the use of the facility where telehealth services are rendered.

Medicaid

In addition to covering provider services at the same rate as in-person services, Medicaid also pays $21.36 per session as an originating site facility fee (Q3014) to cover onsite administrative costs in Colorado.

Medicare

CMHCcs are eligible for Medicare reimbursement in Colorado if they are located in a HPSA (Health Professional Shortage Area).

FQHCs

For Colorado Medicaid, a billable encounter at an FQHC is an in-person face to face visit with a Health First Colorado member. There is no carve out paying fee schedule for telemedicine services. The costs and salaries associated with a telemedicine visit are appropriately included in the cost report, but the service is not a billable encounter. The services are appropriately reimbursed through the prospective payment system by including the costs in the reimbursement calculation.

What tech does my clinic need?

Setting up telepsychiatry at your clinic is simple. The set up will require:

  • A two-way video telecommunication system that allows for real-time communication between the provider and patient
  • HIPAA-compliant video conferencing technology

UTILIZATION & PROGRAM SUSTAINABILITY

Utilization is the percentage of a telepsychiatrist’s time that is spent on reimbursable patient care. Tracking this number helps clinics understand the financial sustainability of telepsychiatry for their organization.
